Documents to gather in preparation for registration
1. You will be required to provide TDLL with a copy of your child’s Birth Certificate during registration. TDLL athletes who participated in TDLL seasons in 2021-2024 will likely already have it on file.
2. You will be required to submit an official school enrollment form. Please request your child’s school complete the School Enrollment Form. If you provided an official school enrollment form in 2022, 2023 or 2024, and your child hasn't changed schools, you can skip this item. If you are new to TDLL, please print this form and give to your school's registrar or secretary.
3. You will be required to complete and submit a Medical Release form (English/Spanish) for each athlete in your household.
You may return completed forms to Big 5 Sporting Goods or bring to Wahtonka Gym during Assessments on February 15th and 16th. Big 5 will have blank copies of the School Enrollment form and Medical Release form. DO NOT EMAIL THESE FORMS.
Other agreements & forms to be e-signed during registration
1. Concussion Information
3. Parent Code of Conduct
4. Player Code of Conduct
Code of Conduct - Each athlete and parent will be required to read and agree to our Code of Conduct.
Volunteering - TDLL is successful because of our dedicated volunteers. All families will be required to opt-in to a volunteer opportunity during the season. We have plenty of volunteer opportunities to choose from including coaching a team, umpiring games or working in the concession stand. Volunteering is a requirement of eligibility. TDLL will need approximately 100 coaches in order to run a successful program this season.
Fundraising - TDLL will hold one fundraiser during the regular season. All athletes (meaning each athlete in a multi-athlete family) are expected to participate in the fundraiser or you can pay to opt-out ($40/athlete). Fundraiser participation is a requirement of eligibility.
